Switching to My Utah Pharmacy is easy! To have your prescriptions transferred from another pharmacy, please select the pharmacy you would like to have them transferred to and fill out the form below.
Our staff will take care of the rest – we will contact your current pharmacy directly to handle the transfer on your behalf.
What you will need: Your current pharmacy name, the drug name, strength and quantity, and your doctor information.
